A case of 18-year-old female patient, height 154 cms, weight 106 kg, BMI 44.68 PCOS/hyperthyroidism, no other comorbidities, uneventful surgery, discharged in 48 hours. Presented on POD15 with fever CT detected a sub-diaphragmatic abscess with no leak of contrast. Exploration revealed no obvious site of leak drainage was performed. This video demonstrates laparoscopic conversion of a sleeve leak to a Roux-en-Y gastric bypass. Suture closure of perforated sleeve, creation of gastric pouch, completed anastomosis, horizontal stapling of sleeve to create gastric pouch, and completed Roux-en-Y gastric bypass conversion of sleeve leak are describes in details. Diversion of the high pressure lumen has to be done to allow late fistulas to heal. Decompression of the high intra-gastric pressure within the sleeve to a low pressure system and a Roux limb allows for better drainage than a sleeve, with possible functional disorders or stenotic areas.
