Clinical Efficacy of Ayurvedic formulations, Kanakasava and Trivrit Churna, in the Management of Bronchial Asthma: A Prospective, Open-label, Multicentric Study

Abstract

Background: Bronchial asthma is a respiratory disorder characterized by chronic airway inflammation and airway hyperresponsiveness leading to symptoms such as wheezing, cough, and dyspnea. While exploring Ayurveda classics, the disease Tamaka Shwasa mentioned in Ayurveda is comparable to the symptomatology of bronchial asthma. Objectives: To assess the clinical efficacy and safety of Kanakasava and Trivrit Churna in the management of bronchial asthma. Materials and methods: A prospective, open-label multicentric, single-arm study was carried out in three peripheral institutes of Central Council for Research in Ayurvedic Sciences (CCRAS). A total 210 participants diagnosed with bronchial asthma (Tamaka Shwasa) and fulfilling the inclusion criteria were enrolled. Each participant was administered with Kanakasava 20 mL twice daily with equal amount of water after meals and Trivrit Churna 3 g once at bed time with lukewarm water for 12 weeks. Follow-up for additional 2 weeks without any medication after the completion of the trial was done. Parameters such as changes in peak expiratory flow rate (PEFR) and forced expiratory volume in one second (FEV1%), Asthma Control Questionnaire Score, and St George's Respiratory Questionnaire (SGRQ-C) were assessed. The statistical analysis (paired t-test) was done to find out the efficacy of the trial drugs before and after intervention. Results: Statistically significant changes were found in PEFR and FEV1 values: PEFR value had improved from 357.637 (107.56) to 394.708 (108.36), and FEV1 value improved from 84.25 (5.45) to 89.48 (10.74) at the end of follow-up. Similarly significant changes were found in Asthma Control Questionnaire (p value < 0.001) and all the four domains of St. George Respiratory Questionnaire when compared to the baseline data. Conclusion: Kanakasava and Trivrit Churna were found to be effective in providing symptomatic relief to the patients with bronchial asthma.

Clinical Evaluation of Sukumara Ghritam and Brahmi Churnam in the Management of Menopausal Syndrome: A Prospective Open-label Study

Abstract

Introduction: Menopause (Rajonivritti) is one of the most significant events in a woman's life and brings a lot of physiological changes that affect the life of a woman permanently. Ayurveda consider it as a normal physiological process and hence have not included it as a gynecological disorder. Objective: To evaluate the clinical efficacy and safety of Sukumara Ghritam and Brahmi Churnam in the management of menopausal syndrome in women. Materials and methods: This trial was a single-center, single arm study conducted on 49 women aged between 40 years and 55 years having amenorrhea ≥12 months; Kupperman menopausal index score ≥15; follicle-stimulating hormone (FSH) level ≥20 IU/L; and endometrial thickness ≤5 mm. Ayurvedic classical formulations, Sukumara Ghritam (6 g) 1 hour before food and Brahmi Churnam (3 g) after food were administered orally twice a day with lukewarm water for 84 days, and subsequent 28 days follow-up was without intervention. Outcome, i.e., changes in the score of menopause rating scale (MRS) and improvement in the quality of life using menopause-specific quality of life (MENQOL) questionnaire, were assessed at baseline, 84th day, and 112th day. Paired sample t test was used to compare mean change from baseline to 84th day and 112th day. p value < 0.05 was considered significant. Results: The results reveal that the effect of therapy in decreasing MRS total score was statistically significant (p value < 0.001). The mean score at the baseline was 7.14 (7.47), at 84th day 4.55 (2.67), and 3.33 (2.53) at 112th day (follow-up period without trial interventions). The improvements in MENQOL in four domains, namely, vasomotor, psychosocial, sexual, and physical, were also significant (p value < 0.001) at 84th day and 112th day in comparison to the baseline. All the safety laboratory parameters such as liver and renal function tests remained within the normal limits throughout the trial period, and no adverse events/adverse drug reaction were reported. Conclusion: Sukumara Ghritam and Brahmi Churnam are found to be safe and effective in the management of menopausal syndrome.

Evaluation of the Effect of Jambira Pinda Sweda in Cervical Spondylosis w.r.t. Clinical Symptomatology

Abstract

Introduction: Cervical spondylosis is a chronic degenerative condition of the cervical spine that affects the vertebral bodies, intervertebral discs of the neck, and the contents of the spinal canal. Spondylosis progresses with age and often develops at multiple interspaces. Though many conventional treatments are available but in view of their side effects and dependency, it is important to search for safe and effective Ayurvedic treatment. Objective: To evaluate the effect of Jambira Pinda Sweda in cervical spondylosis w.r.t. clinical symptomatology. Materials and methods: A single-arm, open, prospective interventional clinical study was conducted at two peripheral institutes of Central Council for Research in Ayurvedic Sciences. Total 60 patients of either sex aged between 30 years and 65 years, with clinical and radiological evidence of cervical spondylosis, were selected for the study. The treatment procedure Jambira Pinda Sweda was done daily for the period of 14 days. The patients were assessed at baseline, 7th day, 14th day, and 21st day (after 1 week without intervention) on the basis of relief in sign and symptoms of cervical spondylosis by reduction in the visual analog scale (VAS) score, Northwick Park Neck Pain Questionnaire, and SF-36 Health Survey Score for Quality of life. Results: Mean value of VAS on baseline was 6.90, on 7th day it was 3.37, and decreased up to 2.57 on 14th day and 2.08 on 21st day showing statistically significant improvement (p < 0.001). The mean value of Northwick Park Neck Pain Questionnaire on baseline was 45.79, on 7th day it was 23.57, which further decreased to 17.32 on 14th day showing statistically significant decrease (p < 0.001). The significant improvement was revealed in all the parameters of the SF-36 health survey (p < 0.001). Conclusion: Jambira Pinda Sweda have been shown to be an effective local Ayurvedic treatment for cervical spondylosis. During the study, no adverse effect was noticed; hence, it proved to be a safe treatment.

Randomized Placebo-controlled Double-blind Clinical Trial on the Efficacy of AYUSHMANAS in the Management of Smritidaurbalya (Cognitive Deficit): A Study Protocol

Abstract

Background: According to modern medicine, individuals whose general intellectual functioning is significantly below average and concurrently having deficits in adaptive behavior are considered to be having cognitive deficit. It may be congenital or caused by factors such as brain injuries, neurological disorders, or mental illness. In Ayurveda, the condition can be correlated with Manasamandata. Various terms are used in the classics like Buddhimandyata, Jadata, Smritidaurbalya, etc., to denote impairment of intellectual functions. The current medications for cognitive deficit in modern medicine are not entirely satisfactory, so many patients seek alternative treatment for its management. Thus, there is a definite need to scientifically assess some of alternative treatment modalities. Objective: To assess the efficacy and safety of a coded Ayurvedic drug—AYUSHMANAS—in children suffering from cognitive deficit. Materials and methods: This is a prospective, randomized, double-blind, two-arm placebo-controlled clinical study. A total of 150 participants (75 in each arm) suffering with cognitive deficit will be included in line with inclusion and exclusion criteria. The participants of trial group will be administered with AYUSHMANAS (250 mg × 2 tablets) while in control group Placebo (250 mg × 2 tablets) will be administered twice daily after food for 180 days. A total of seven visits will be scheduled for each participant: first visit at screening day 0, 30th day, 60th day, 90th day, 120th day, 150th day, and 180th day. The outcomes intended from the study include observing changes in Binet Kamat test of intelligence (BKT) and mini-mental state examination (MMSE) and a modified MMSE (mMMSE) scores apart from assessing safety of the intervention. Observation of therapeutic and safety endpoint outcomes will be undertaken. Discussion: This clinical protocol is designed for exploring a better clinical outcome with good safety profile in the management of cognitive deficit in children. If found effective, the selected drug can be listed in the management protocol of cognitive deficit at clinical practice level in terms of better efficacy, safety, and cost-effective intervention. The selected drug may also lead to better understanding and management of cognition related problems. Trial registration: CTRI/2019/07/020324

Hormonal and Menstrual Change after Modification of Chronotype—An Eye Opener Treatment Strategy: A Case Report

Abstract

In urban areas, especially in adolescents, there is an increasing trend to engage in activities, which are not in compliance with circadian clock and natural rhythm of light dark circle, turning them into a night owl chronotype. Research evidences highlight various health issues as a result of this chronotype. There are no research evidences of whether change of this night owl chronotype to lark shall correct these health issues. This is a unique case of a young unmarried lady, aged 22 years, from Lucknow, India, who visited OPD of Regional Ayurveda Research Institute for Eye Disorders (RARIED), Lucknow, with complaints of scanty menstrual flow along with hormonal imbalance, which had no improvement even after 6–7 months of medication. On probing the cause of these complaints, it was found that the patient had a night owl chronotype since past few years along with untimely food habits. The patient was then counseled for shifting the chronotype to lark along with timely food habits, which is in compliance with Ayurveda Guidelines of Lifestyle (AGOL) without any medications. After that a remarkable improvement in menstrual health was observed. Menstrual flow reached back almost to normal and the hormonal profile was balanced, with an exception to anti-mullerian hormone (AMH), which had only a slight improvement.

An Ayurvedic Approach to Prolapsed Intervertebral Disc (Gridhrasi): A Case Report

Abstract

This case study exemplifies the efficacy of Ayurvedic medications and procedures in the management of prolapsed intervertebral disc (PIVD), where disc bulge with nerve impingement at L5-S1 was diagnosed radiologically. A 24-year-old patient presented to the OPD of Dept. of Kayachikitsa, Ch. Brahm Prakash Ayurved Charak Sansthan, New Delhi, with complaints of severe low back pain radiating to the bilateral lower limb, rendering him unable to do his day-to-day activities. The pain started 8 months back, but the intensity of pain became severe in the last 1 month so much so that he had to rely upon others to do his routine activities and this brought about considerable financial impact also. This case was diagnosed as PVID or lumbar disc herniation at the L5-S1 level based on the magnetic resonance imaging (MRI) report. The patient was treated with Panchakarma procedures and Ayurvedic oral drugs and got satisfactory results with no side effects. The visual analog scale (from 9 to 0), Oswestry disability index (from 58% to 17%), SLR (from 35° left and 30° right to 90° in bilateral limbs), ESR (from 45 to 8), and MRI report were considered as parameters of assessment. Sustained and satisfactory improvement was noted at the follow-up after 2 months and protrusion of disc was resolved during treatment. After the course of treatment, the pain was completely resolved during the walking or rest condition and during the follow-up period, he also resumed his work with no disquiet. This case study validates that PIVD can be successfully treated with the Ayurvedic treatment modality.

Vaidyasaramu (Siddhavaidyasaramu/Navanatha Siddha Saramu) of Rayasam Peraya: A Classical Ayurvedic Text of Andhra Region Written in Classical Telugu Verses

Abstract

Background: Rayasam Peraya's Vaidyasaramu is a valuable Ayurvedic text written in Telugu verses. It is a popular Ayurvedic text with simple and unique formulations. In the old Andhra region, the Siddha tradition was very popular. Nagarjuna and other Siddha physicians, belonging to this place, influenced Ayurvedic scholars of this region, and they became well versed in using mercury, poisons, and other metals, minerals, gems, etc., in purified and incinerated form along with herbal drugs. The present text represents such Ayurvedic tradition mingled with the Siddha tradition. Aim: To highlight the content of Rayasam Peraya's Vaidyasaramu and draw the attention of scientific fraternity for further research and utilization of its unique formulations. Review results: Unique formulations with clinical significance are highlighted in this paper. Tiktakoshataila, Hanumantataila, Angarakataila, Kadalikandaghrita, Ramasitadi Churna, Ashvattadi Churna, Hayavati Churna, Vamanadi Churna, etc. are some examples. Certain variations in disease descriptions in comparison to popular Ayurvedic texts are also highlighted. Conclusion: This text comprises the author's formulations and compilation of certain popular Ayurvedic texts. This medical text described in Telugu Chhandas verses is our pride to show that we had our beautiful past which is forwarded to the future with intensified fragrance. Research on unique formulations may give leads to clinical studies. Clinical significance: The text describes purification and incineration of many metals, mercury, gems, poisonous substances, and their usage in various diseases. Medically, it is an important text because of unique, simple, and useful formulations with clinical significance. Certain disease descriptions like the types of diseases mentioned in this text are different in comparison to popular classical Ayurvedic texts like Brihatrayi and Laghutrayi. Further study may reveal new diseases in comparison to disease descriptions of contemporary medical sciences.
